Participation Paperwork

Paperwork Information: All students must turn in the following paperwork and receive a “yellow card” from the Athletic Department before participating on the team.

Students must complete the following steps prior to the first practice in order to receive the required yellow card for participation:

• Print and complete (with the assistance of a medical professional)  the VHSL Pre-participation Exam Form. Scan and email this form directly to the Activities Office: dlreynolds1@fcps.edu.  Additional information is below.
• Complete the West Potomac High School Athletic Registration  
• Download and complete an Emergency Care Card. Email this form to your coach or bring it to the first practice.

Please also complete the following forms and turn in to your Rifle Coach:

• FCPS Field Trip Driver's License and Vehicle Insurance Information: This is for parents to be able to drive students to the matches. Even if you don't think you'll be driving, please complete and submit the form. In the "Destination" field, enter "DC/MD/VA", and in the "Purpose" and "Explain" fields, enter "Rifle team match". 
• Rifle Team Policies: Read and acknowledge by email (see instructions on document)
• Izaak Walton Student Release Statement: Print and complete

Students must have a completed physical exam prior to participating in  high school sports.  The exam must be completed by a MD, DO, NP or PA  on the current VHSL form.  The form must be completed by the physician  after May 1, 2021 to be valid for the 2021-22 school year.  Completed  forms should be turned in  directly to the Activities Office (dlreynolds1@fcps.edu).  Complete all four pages.  Parent completes pages 1 and 4 and a doctor completes pages 2 and 3.
