Participation Paperwork


Required Paperwork


Students and parents will need to complete several steps prior to participating in high school sports, including:

  • FCPS Participation Policy: A completed VHSL Pre-Participation Exam Form (must be on the VHSL form and completed after May 1, 2018 in order to be valid for the 2018-19 school year)
  • Parent Concussion Education (online through FCPS)
  • Student Concussion Education (online through FCPS)
  • FCPS Emergency Care Information: An Emergency Care Card (available as part of FCPS General Registration packet)
  • A completed Student Participation Pledge (will be available before the first contest date of fall season)
  • FCPS Field Trip Driver's License and Vehicle Insurance:  This is for parents to be able to drive students to the matches. Even  if you don't think you'll be driving, please complete and submit the  form. For destination fill in DC/MD/VA and for Purpose and Explain fill  in Rifle team match.

Virginia High School League Pre-participation Exam Form

Students must have a completed physical exam prior to participating in  high school sports.  The exam must be completed by a MD, DO, NP or PA  on the current VHSL form.  The form must be completed by the physician  after May 1, 2018 to be valid for the 2018-19 school year.  Completed  forms should be turned in to the Activities Office at school.  Complete all four pages.  Parent completes pages 1 and 4 and a doctor completes pages 2 and 3. 


Concussion Education--Parents AND Students

Students AND Parents must complete the concussion education program prior to the  student’s participation in high school sports.  Parents need only  complete the program one time for all their children participating in  FCPS athletics if they register each child's FCPS ID number prior to  viewing the slideshow.
